Medication

No student shall be given medication during school hours except upon written request from the parent/guardian of the pupil AND a licensed physician who has the responsibility for the medical management of the pupil (Educational Code Section 49423).

Medication includes prescription AND over the counter pills, drops, inhalants, lotions, ointments, and injections.

If medication is to be taken at school, your physician must complete and sign the Request for Medication to be Taken in School Form. 

If there is any change in medication, dosage or schedule, a new request form must be submitted.

At the end of the school year, parents must claim any medication remaining at the school.
